Why have I been invited for a long term condition monitoring appointment?

All patients with a long term condition are invited in their month of birth for a review. Examples of these conditions include Diabetes, Asthma, COPD, Stroke/TIA and Heart Failure.

What happens at a review?

Depending on your underlying condition this may include a blood test, blood pressure check and weight measurement. Diabetic patients are usual asked for a urine sample. Patients with breathing conditions should bring their inhalers with them. 

Who will I see at my appointment? Will I see a GP?

Our experienced nursing team and clinical pharmacist complete the majority of these reviews. Depending on the results you will be contacted to have a follow up consultation with the GP if required.
